Senior Case Manager - care planning (Austin, TX)

Northwind Forge Care Services provides in-home and community support in Austin, and we need a Senior Case Manager to lead our care planning. You would join a small organisation of about 35 people based in Austin. What you will do • Help with applications and appointments for client records. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Austin, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
