Senior Case Manager - care planning (Austin, TX)

We are growing our team at Harborlight Stone Care Services Partners and adding a Senior Case Manager whose main responsibility will be care planning. You would join a small organisation of about 38 people based in Austin. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for family communication. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Austin. • We will tell you what to expect at each step of the hiring process.
