Senior Case Manager - care planning (Austin, TX)

Summit Mill Care Services Partners supports families and individuals in Austin with personal care and coordination, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish.
