Senior Case Manager - care planning (Atlanta, GA)

Westbrook Cobalt Learning Group operates schools and learning programmes around Atlanta, and we need a Senior Case Manager to lead our care planning. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for school calendars. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with school calendars. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Atlanta. • Applications are reviewed on a rolling basis.
