Senior Case Manager - care planning (Anchorage, AK)

Ironwood West Care Services supports families and individuals in Anchorage with personal care and coordination. We are hiring a Senior Case Manager to take on care planning. You would join a large organisation of about 972 people based in Anchorage. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for caregiver training. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Anchorage. • We reply to every applicant.
