Senior Case Manager - care planning (Anchorage, AK)

We are growing our team at Westbrook Juniper Civic Services Group and adding a Senior Case Manager whose main responsibility will be care planning. We follow procedures carefully and explain them clearly. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for permits. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with permits. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Anchorage. • Applications are reviewed on a rolling basis.
