Senior Case Manager - care planning (Anchorage, AK)

Clearpath Cross Civic Services supports public agencies in Anchorage with budgeting, records and programmes. We are hiring a Senior Case Manager to take on care planning. We follow procedures carefully and explain them clearly. What you will do • Help with applications and appointments for case files. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with case files. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Anchorage, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
