Senior Case Manager - benefits navigation (Tucson, AZ)

Silverline Forge Care Services Group provides in-home and community support in Tucson, and we need a Senior Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Full-time, on a regular day shift. • On site in Tucson, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
