Senior Case Manager - benefits navigation (Tampa, FL)

Harborlight Mill Care Services Partners is a social-care provider serving older adults and people with disabilities around Tampa. We are hiring a Senior Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Tampa, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
