Senior Case Manager - benefits navigation (Seattle, WA)

Rivermark Rowan Care Services is looking for a Senior Case Manager. The job centres on benefits navigation for care plans, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for care plans. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Seattle. • Applications are reviewed on a rolling basis.
