Senior Case Manager - benefits navigation (Seattle, WA)

Summit Lakeside Health Partners is a healthcare provider in Seattle with clinics and support teams. We are hiring a Senior Case Manager to take on benefits navigation. You would join a small organisation of about 22 people based in Seattle. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for three months, with the possibility of becoming permanent. • On site in Seattle. • We reply to every applicant.
