Senior Case Manager - benefits navigation (Rochester, NY)

Silverline Union Health Group provides primary and specialty care in Rochester. We are hiring a Senior Case Manager to take on benefits navigation. We are busy and kind, and we look after each other. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Rochester. • Applications are reviewed on a rolling basis.
