Foxglove Umber Health is looking for a Senior Case Manager. The job centres on benefits navigation for clinic supplies, and it would suit someone who likes steady, practical work. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with clinic supplies.
