Senior Case Manager - benefits navigation (remote)

Bluepeak Bridge Health Group provides primary and specialty care in Memphis, and we need a Senior Case Manager to lead our benefits navigation. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Connect clients with partner agencies. • Help with applications and appointments for clinic supplies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with quarterly visits in Memphis. • We will tell you what to expect at each step of the hiring process.
