Senior Case Manager - benefits navigation (remote)

Summit Junction Community Partners is a Rochester charity working with residents on basic needs and opportunity. We are hiring a Senior Case Manager to take on benefits navigation. Mission matters here, and so does looking after staff. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for grant reporting. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Part-time, about 20 hours a week, afternoons. • Remote-friendly, with quarterly visits in Rochester. • We will tell you what to expect at each step of the hiring process.
