Senior Case Manager - benefits navigation (remote)

Bluepeak Stone Care Services supports families and individuals in Louisville with personal care and coordination. We are hiring a Senior Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Part-time, about 16 hours a week, mornings. • Remote-friendly, with a monthly team day in Louisville. • We will tell you what to expect at each step of the hiring process.
