Senior Case Manager - benefits navigation (remote)

Stonegate Mill Care Services Group is a social-care provider serving older adults and people with disabilities around Philadelphia. We are hiring a Senior Case Manager to take on benefits navigation. Teams support each other, especially on hard days. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for licensing. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in Philadelphia. • Applications are reviewed on a rolling basis.
