Senior Case Manager - benefits navigation (remote)

Fernwood Keystone Care Services supports families and individuals in Cleveland with personal care and coordination. We are hiring a Senior Case Manager to take on benefits navigation. The work is personal and so is the culture. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client records. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with client records. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with a monthly team day in Cleveland. • We will tell you what to expect at each step of the hiring process.
