Senior Case Manager - benefits navigation (remote)

Ironwood Ivory Health runs clinics and patient services around Honolulu. We are hiring a Senior Case Manager to take on benefits navigation. Continuing education is supported. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for electronic records. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 16 hours a week, spread across three days. • Remote-friendly, with quarterly visits in Honolulu. • We reply to every applicant.
