Senior Case Manager - benefits navigation (remote)

Greenridge Quill Health runs clinics and patient services around Fort Worth. We are hiring a Senior Case Manager to take on benefits navigation. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for clinic supplies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Contract for six months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Fort Worth. • Applications are reviewed on a rolling basis.
