Senior Case Manager - benefits navigation (remote)

Clearpath Vista Community Partners is a Rochester charity working with residents on basic needs and opportunity. We are hiring a Senior Case Manager to take on benefits navigation. You would join a large organisation of about 827 people, working mostly from home. What you will do • Help with applications and appointments for donor records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Rochester. • We will tell you what to expect at each step of the hiring process.
