Senior Case Manager - benefits navigation (remote)

Cedar Ivory Care Services provides in-home and community support in Fort Worth, and we need a Senior Case Manager to lead our benefits navigation. The work is personal and so is the culture. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with quarterly visits in Fort Worth. • We will tell you what to expect at each step of the hiring process.
