Senior Case Manager - benefits navigation (remote)

Stonegate Beacon Community Partners is looking for a Senior Case Manager. The job centres on benefits navigation for client intake, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Rochester. • Applications are reviewed on a rolling basis.
