Senior Case Manager - benefits navigation (remote)

Stonegate Forge Care Services is a social-care provider serving older adults and people with disabilities around Nashville, and we need a Senior Case Manager to lead our benefits navigation. You would join a small organisation of about 25 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for licensing. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, with flexible days. • Fully remote, with quarterly visits in Nashville. • Applications are reviewed on a rolling basis.
