Senior Case Manager - benefits navigation (Philadelphia, PA)

We are growing our team at Nova Quill Health and adding a Senior Case Manager whose main responsibility will be benefits navigation. You would join a large organisation of about 203 people based in Philadelphia. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for patient education. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Philadelphia. • We will tell you what to expect at each step of the hiring process.
