Senior Case Manager - benefits navigation (Oklahoma City, OK)

Summit Hollow Care Services provides in-home and community support in Oklahoma City, and we need a Senior Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Oklahoma City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
