Senior Case Manager - benefits navigation (Nashville, TN)

We are growing our team at Westbrook Stone Health Group and adding a Senior Case Manager whose main responsibility will be benefits navigation. You would join a small organisation of about 30 people based in Nashville. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish.
