Senior Case Manager - benefits navigation (Memphis, TN)

Greenridge Juniper Health Group is a healthcare provider in Memphis with clinics and support teams, and we need a Senior Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Part-time, about 24 hours a week, mornings. • On site in Memphis. • We will tell you what to expect at each step of the hiring process.
