Senior Case Manager - benefits navigation (Memphis, TN)

Bluepeak Bridge Health Group runs clinics and patient services around Memphis. We are hiring a Senior Case Manager to take on benefits navigation. Clinicians and administrators work side by side. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Memphis. • We reply to every applicant.
