Senior Case Manager - benefits navigation (Memphis, TN)

Greenridge Mill Health provides primary and specialty care in Memphis. We are hiring a Senior Case Manager to take on benefits navigation. You would join a large organisation of about 768 people based in Memphis. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Memphis, with some flexibility after the first few months. • We reply to every applicant.
