Senior Case Manager - benefits navigation (Memphis, TN)

Northwind Stone Health Group provides primary and specialty care in Memphis, and we need a Senior Case Manager to lead our benefits navigation. Continuing education is supported. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for insurance authorisations. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Memphis. • We will tell you what to expect at each step of the hiring process.
