Senior Case Manager - benefits navigation (Memphis, TN)

We are growing our team at Stonegate Yarrow Care Services and adding a Senior Case Manager whose main responsibility will be benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Memphis. • Applications are reviewed on a rolling basis.
