Senior Case Manager - benefits navigation (Louisville, KY)

We are growing our team at Lumen Fox Community Partners and adding a Senior Case Manager whose main responsibility will be benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for benefits enrolment. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with benefits enrolment. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Louisville. • We will tell you what to expect at each step of the hiring process.
