Senior Case Manager - benefits navigation (Little Rock, AR)

We are growing our team at Clearpath Stone Health and adding a Senior Case Manager whose main responsibility will be benefits navigation. Clinicians and administrators work side by side. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, spread across three days. • On site in Little Rock. • We will tell you what to expect at each step of the hiring process.
