Senior Case Manager - benefits navigation (Kansas City, MO)

Foxglove Quill Care Services is a social-care provider serving older adults and people with disabilities around Kansas City. We are hiring a Senior Case Manager to take on benefits navigation. The work is personal and so is the culture. What you will do • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Kansas City. • We reply to every applicant.
