Senior Case Manager - benefits navigation (Jackson, MS)

Foxglove Cedar Health is looking for a Senior Case Manager. The job centres on benefits navigation for patient scheduling, and it would suit someone who likes steady, practical work. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for patient scheduling. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, spread across three days. • On site in Jackson. • We will tell you what to expect at each step of the hiring process.
