Senior Case Manager - benefits navigation (Houston, TX)

We are growing our team at Amberfield Umber Health Partners and adding a Senior Case Manager whose main responsibility will be benefits navigation. You would join a large organisation of more than 1,000 people based in Houston. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for referrals. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 20 hours a week, afternoons. • On site in Houston. • We reply to every applicant.
