Senior Case Manager - benefits navigation (Hartford, CT)

Westbrook Sable Community Partners runs community programmes across Hartford, and we need a Senior Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for volunteer shifts. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish.
