Senior Case Manager - benefits navigation (Hartford, CT)

Greenridge Wren Care Services Group supports families and individuals in Hartford with personal care and coordination. We are hiring a Senior Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Hartford. • Applications are reviewed on a rolling basis.
