Senior Case Manager - benefits navigation (Grand Rapids, MI)

We are growing our team at Fernwood Field Community Partners Group and adding a Senior Case Manager whose main responsibility will be benefits navigation. Flexibility and kindness are expected. What you will do • Help with applications and appointments for donor records. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with donor records. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with the possibility of becoming permanent. • On site in Grand Rapids. • We reply to every applicant.
