Senior Case Manager - benefits navigation (Fort Worth, TX)

Clearpath River Community Partners runs community programmes across Fort Worth, and we need a Senior Case Manager to lead our benefits navigation. We are small enough to know everyone's name. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Fort Worth. • We will tell you what to expect at each step of the hiring process.
