Senior Case Manager - benefits navigation (Cleveland, OH)

Clearpath Forge Care Services supports families and individuals in Cleveland with personal care and coordination. We are hiring a Senior Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Cleveland, with some flexibility after the first few months. • We reply to every applicant.
