Senior Case Manager - benefits navigation (Albuquerque, NM)

We are growing our team at Lumen Cedar Health Group and adding a Senior Case Manager whose main responsibility will be benefits navigation. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • On site in Albuquerque. • Applications are reviewed on a rolling basis.
