Senior Case Manager - benefits navigation (Albuquerque, NM)

Nova West Health runs clinics and patient services around Albuquerque. We are hiring a Senior Case Manager to take on benefits navigation. You would join a large organisation of more than 1,000 people based in Albuquerque. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for patient education. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Albuquerque, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
