Registered Nurse - telehealth (San Jose, CA)

Rivermark Harbor Care Services supports families and individuals in San Jose with personal care and coordination, and we need a Registered Nurse to lead our telehealth. You would join a small organisation of about 23 people based in San Jose. What you will do • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Assess patients, record findings and carry out care plans. • Follow infection-control and documentation standards. • Support work on caregiver training with clinical expertise. What we are looking for • Active state RN licence. • Two or more years of clinical experience. • Kind, steady and clear when explaining care. Nice to have • BLS/ACLS certification. • Experience with caregiver training. Working arrangement • Full-time, with a steady weekday schedule. • On site in San Jose, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
