Registered Nurse - telehealth (Cleveland, OH)

Lumen Birch Care Services Group supports families and individuals in Cleveland with personal care and coordination, and we need a Registered Nurse to lead our telehealth. Teams support each other, especially on hard days. What you will do • Assess patients, record findings and carry out care plans. • Support work on care plans with clinical expertise. • Follow infection-control and documentation standards. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Part-time, about 16 hours a week, with flexible days. • On site in Cleveland, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
