Registered Nurse - patient education (San Jose, CA)

Northwind Sparrow Care Services Group supports families and individuals in San Jose with personal care and coordination, and we need a Registered Nurse to lead our patient education. Teams support each other, especially on hard days. What you will do • Follow infection-control and documentation standards. • Support work on care plans with clinical expertise. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Educate patients and families in plain language. What we are looking for • Kind, steady and clear when explaining care. • Active state RN licence. • Two or more years of clinical experience. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in San Jose. • We reply to every applicant.
