Registered Nurse - patient education (Salt Lake City, UT)

Stonegate West Care Services Partners is a social-care provider serving older adults and people with disabilities around Salt Lake City, and we need a Registered Nurse to lead our patient education. You would work closely with colleagues in operations, support and management. What you will do • Assess patients, record findings and carry out care plans. • Educate patients and families in plain language. • Coordinate with physicians, pharmacists and social workers. • Follow infection-control and documentation standards. • Support work on care plans with clinical expertise. What we are looking for • Kind, steady and clear when explaining care. • Two or more years of clinical experience. • Active state RN licence. Nice to have • Experience with care plans. • BLS/ACLS certification. Working arrangement • Contract for twelve months, with the possibility of becoming permanent. • On site in Salt Lake City. • We reply to every applicant.
