Registered Nurse - care navigation for device patients (Philadelphia, PA)

Foxglove Lantern Medical makes wearable monitoring devices for people with heart conditions, and our nurses help patients understand their readings. We are hiring a part-time registered nurse in Philadelphia who can work remotely. What you will do • Call patients who receive a new device and walk them through setup and what alerts mean. • Review flagged readings with our clinical team and arrange follow-up with their doctors. • Document each call carefully in our care-management system. What we are looking for • Active Pennsylvania RN licence, or a compact licence. • Two or more years in cardiac care, telehealth or care coordination. Part-time, 20 to 24 hours a week, remote-friendly, with some evening coverage.
