Registered Nurse - care coordination (Wichita, KS)

Rivermark Sparrow Transit Planning is a transit-planning firm in Wichita working with agencies and riders. We are hiring a Registered Nurse to take on care coordination. Work is collaborative and map-heavy. What you will do • Follow infection-control and documentation standards. • Assess patients, record findings and carry out care plans. • Coordinate with physicians, pharmacists and social workers. • Support work on service alerts with clinical expertise. • Educate patients and families in plain language. What we are looking for • Active state RN licence. • Kind, steady and clear when explaining care. • Two or more years of clinical experience. Nice to have • Experience with service alerts. • BLS/ACLS certification. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Wichita, with some flexibility after the first few months. • We reply to every applicant.
